Quadriceps Tendon Rupture Repair

The quadriceps tendon connects the large thigh muscles (the quadriceps) to the kneecap (patella), allowing you to straighten your leg and perform essential movements like walking, standing, and climbing stairs. A rupture of this tendon is a serious injury that can severely limit mobility and function.
  • A quadriceps tendon rupture typically occurs from a sudden forceful contraction of the thigh muscles, such as during a fall or landing awkwardly from a jump.

It is more common in men over 40 and in individuals with certain chronic conditions or previous knee problems. Patients often report a popping sensation, followed by pain, swelling, and an inability to extend the leg at the knee.

  • This graft is becoming an increasingly popular choice among athletes and active individuals because it offers:

    • Strong, Durable Graft – Thicker than patellar or hamstring tendons, ideal for high-demand knees

    • Lower Re-Rupture Risk – Especially in younger or high-performance athletes

    • Reduced Anterior Knee Pain – Compared to patellar tendon autografts

    • Excellent Healing Potential – Because it’s your own tissue

    • Option for Revision Surgery – Often used when other grafts have already been used

Quadriceps tendon rupture repair is typically performed within days or weeks of the injury. Delaying treatment may make the repair more complex. During surgery, Dr. Romanelli reattaches the torn tendon to the kneecap using strong sutures and anchors, restoring the integrity of the extensor mechanism of the knee.

This outpatient procedure is performed under anesthesia and usually takes about one to two hours.

  • Dr. Romanelli may recommend a quadriceps tendon autograft for:

    • Athletes requiring a strong, stable ACL reconstruction

    • Patients undergoing revision ACL surgery

    • Individuals with a history of patellar tendon pain

    • Those looking for an autograft option with robust long-term outcomes

  • The procedure is performed arthroscopically through small incisions. Dr. Romanelli harvests the quadriceps tendon, prepares the graft, and replaces the damaged ACL with precision-guided techniques. Most patients go home the same day and begin rehabilitation soon after.

  • Recovery timelines vary, but most patients can expect:

    • Crutches for 1–2 weeks

    • Physical therapy beginning immediately

    • Light activity at 3–4 months

    • Return to sports between 6–9 months, depending on progress

Dr. Romanelli works closely with physical therapists and athletic trainers to ensure a safe and effective return to activity.
